EVALUATION FORMS FOR RESIDENTS TO COMPLETE
2024-2025 Faculty Shift Evaluation
For student to complete re: Faculty after clinical shifts
Must complete at least 3 per block
i.e. Casting, Simulation, Ultrasound, ECG Teaching, Radiology on Thursdays
Must be completed for each session attended
NEW 2024-2025 End of Rotation Evaluation/Feedback
Must be completed in order for your rotation to be complete
END OF ROTATION CHECKLISTS
RESIDENTS: END OF ROTATION CHECKLIST
For all residents completing a rotation, the following MUST be completed before your rotation is complete.
Complete the clinical shifts assigned with faculty completing a student evaluation form for each shift.
Complete your Observed History and Physical
Complete the MSH ED End of Rotation Evaluation
Complete your U of T Power Rotation Evaluation
Note: this is different from our internal end of rotation evaluation as we do not get to see the evaluations until a much later date. In order to continue adapting to our students needs in a more timely manner, we ask that you please complete both End of Rotation evaluations.
Complete all the Thursday Teaching Session Evaluations for sessions attended
Complete at least three (3) Faculty Shift Evaluations
Then send a final email to Sharona (erdoctors.MSH@sinaihealth.ca) that all of this has been completed
CLERKSHIP: END OF ROTATION CHECKLIST
For all clerks completing a rotation, the following MUST be completed before your rotation is complete.
Complete the clinical shifts assigned with faculty completing a shift evaluation form on Medsis for each shift.
Complete your Observed History and Physical
Complete the EMR310Y Course Evaluation on Medsis
Complete all teaching session evaluations for EM BootCamps (Medsis) and any Thursday Teaching Session Evaluations that you have attended
Complete Faculty Evaluations on Medsis
